Nomination forms are listed below. Click on the appropriate form to submit your nominations.

Coaches of nominated players must have joined TGCA by the October 15th deadline to qualify for these state honors.

As well, any coach nominated for All-Star Coach and/or Coach-of-the-Year must have joined TGCA by the October 15th deadline.

A confirmation email will be sent to you upon receipt of your form. Therefore, please make sure your email address is entered correctly.

ALL nomionation forms are due on the Monday before the state basketball tournament (12:00 noon).
